39 REGISTRATION

Waiver And Release of All Claims and Assumption of Risk

Registration Information You may register online, by phone with Annual Waiver, or in person at Civic Center or Boulder Point. Registrations will be taken first-come, first served. Some programs for kids are designated for particular grades. Please register your child according to the grade he or she is enrolled in for the 2023-2024 school year. If your child has a life-threatening allergy, we recommend you read the allergy information and provide the Park District with all informa tion, authorization and waiver/release forms prior to registration. Registration will be accepted for available classes up until the second meeting of the program at the full program fee. Registration requests after the second class will be put on a waitlist. There is a minimum and maximum enrollment for each class. When class enrollment reaches its maximum a wait list is created. Names on wait lists will be called if an opening becomes available or a new class is scheduled. If a class is canceled due to low enrollment you will be notified and refunded the class fee. Payment & Receipt Payment must be made at the time of registration. VISA, American Express, Discover, and Mastercard will be accepted as payment for online and telephone registrations. When paying by check, please in clude your Driver’s License number on the check or registration form. There will be a $25 service charge for checks returned for reasons other than bank error. When you register for a class online, you will print your own receipt/ confirmation. If you register in person, you will be given a receipt/ confirmation, and when you mail a registration, your receipt/confir mation will be mailed to you. Each receipt/confirmation lists program name, times, days, dates, “not dates” and locations. Please check this information carefully as it may include information that changed after the catalog was printed. Please call the Park District at 630.554.1010 if you have any questions. Refunds If you are unable to attend a class that you registered for, please notify us at least 3 business days before the start date and we will be happy to refund your registration fee. For requests made less than 3 business days prior to the start of the program or event, no transfer or refund will be given. Refund requests made at least 3 days prior to the program start date will be credited to your account. If you request the refund be issued to a credit card or as a check, a $5 service fee per program will be charged.

Please read this carefully and be aware that in signing up and par ticipating in this program/activity, you will be expressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages or loss which you or your minor child/ward might sustain as a result of participating in any and all activities connected with and as sociated with this program (including transportation services/vehicle operation, when provided). I recognize and acknowledge that there are certain risks of physi cal injury to participants in this program/activity, and I voluntarily agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that my minor child/ward or I may sustain as a result of said participation. I further agree to waive and relinquish all claims I or my minor child/ward may have (or accrue to me or my child/ward) as a result of participating in this program/activity against the Oswegoland Park District, including its officials, agents, volun teers, and employees (hereinafter collectively referred to as Oswe goland Park District). I do hereby fully release and forever discharge the district from any and all claims for injuries, damages or loss that my minor child/ward or I may have or which may accrue to me or my minor child/ward and arising out of, connected with, or in any way associated with this program/activity. Warning of Risk: Recreational programs and activities are intended to challenge and engage the physical, mental, and emotional resources of each par ticipant. Despite careful and proper preparation, instruction, medical advice, conditioning, and equipment, there is still a risk of serious injury when participating in any recreational program. Understand ably, not all hazards and dangers can be foreseen. Depending on the particular activity, participants must understand that certain risks, dangers, and injuries due to inclement weather, slipping, falling, poor skill level or conditioning, carelessness, horseplay, unsportsmanlike conduct, premises defect, inadequate or defective equipment, inad equate supervision, instruction or officiating, and all other circum stances inherent to indoor, outdoor, and water related recreational programs exist. In this regard, it must be recognized that it is impos sible for the Oswegoland Park District to guarantee absolute safety. Photo/Video Authorization: Photos and video footage are periodically taken of people participat ing in a Park District program or activity, attending a class or event, or using District facilities or property. Please be aware that by register ing for a program or class, participating in an activity, attending an event, or using District facilities or property, you authorize the District to use these photos and video footage for promotional purposes in District publications, advertising, marketing materials, brochures, event flyers, social media (including Facebook, YouTube, Instagram, Twitter, and other social media sites operated by the District), and the District’s website without additional prior notice or permission and without any compensation to you. All photos and videos are property of the District.
